POSITION SUMMARY

Reporting to the Director of Program Compliance and Operation, the entry level Case Manager coordinates services for all Haven of Graces residents (Women’s Center, Quad Apartments and After Care) who need pre and post-natal housing, health care, financial literacy and job training. This position is full time and exempt. Pay: $36,000.00 – $38,000.00 per year

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Focuses on resident’s needs as it related to (Health Care, WIC, Food Stamps, Job Engagement, etc.)
  • Monitor and track outcomes of above required services
  • Identify and track pertinent program outcomes
  • Pre/Post-Natal
  • Healthy Birth Weight (Zip Code)
  • Mental Health
  • Education
  • Employment
  • After Care
  • Develops and implements comprehensive residents’ goals and develop case plans in coordination with the therapist
  • Arrange and track transportation for residents to connect with resources
  • Assists and supports program outcomes and evaluation
  • Ensure all residents participate in all required community training
  • Tracks, reviews and ensures accurate record keeping and provides timely monthly reports
  • Meets with residents on a weekly basic and document in HMIS database
  • Meets with After Care residents on a monthly basic and document in HMIS database
  • Determine organizational needs (data and systems) to track After Care residents progress
  • Assist with growing, expanding and retaining After Care residents
  • Ability to read and interpret data to determine program success’s
  • Participates as a collaborative member of the program team
  • Actively participates in community resource planning and related work to establish and foster strong partnerships
  • Is a positive ambassador for The Haven of Grace

 

QUALIFICATIONS AND EXPERIENCE

  • Entry Level position
  • Bachelor’s degree required in social work or human services or related field (MSW preferred)
  • 2 years’ experience in case management preferred
  • 2 years’ experience and competencies working in a multiracial, multicultural residential environment preferred
  • HMIS Trained and Proficient
  • Proficiency in Microsoft Office Suite
  • Must have own vehicle and possess a current, valid driver’s license and meet The Haven of Grace requirements for insurance
  • Must be willing to be on call and able to work evening, overnight, weekends and holidays if necessary
  • Pass all required background screenings

WORK ENVIRONMENT:

Most of the work for this position involves transporting and engaging clients in community resources. Some work is performed within the residential setting with crisis management being a key component. The employee will need to travel to Haven related trainings & meetings, events, and other Haven related destinations in personal vehicle; and upon submission of proper documentation will be reimbursed for mileage at current agency rate. Employee must be present at all specified events and is required to have reliable personal transportation.
